Enroll as a COVID-19 vaccine provider
Pharmacies that do not participate in the federal allocation program are encouraged to be part of the solution and should coordinate with their jurisdiction’s health department to become COVID-19 vaccine providers. View the following information before taking steps to enroll as a COVID-19 vaccination provider.

Steps to enroll with your jurisdiction to access and administer a COVID-19 vaccine
Step 1
Work with your state pharmacy association
Your state’s pharmacy association may have up-to-date information on state requirements, data collection efforts, registration as a COVID-19 vaccine provider and more.
The U.S. Pharmacist website has compiled useful links to each state pharmacy association.
Step 2
Sign the CDC Covid-19 vaccination provider agreement
Your jurisdiction’s immunization program will provide guidance on how to access, complete, and submit the Provider Agreement.
Step 3
Enroll in your jurisdiction’s Immunization Information System (IIS)
Immunization information systems (IIS) are confidential, population-based, computerized databases that record all immunization doses administered by participating providers to persons residing within a given geopolitical area.
Your jurisdiction will provide directions for how to enroll.
Step 4
Be trained in proper vaccine administration & management
To enroll as a COVID-19 vaccine provider, the CDC is requiring proper training on how to administer vaccines, report and manage vaccine inventory, store and handle vaccine, report adverse events, report doses administered data, and other essential topics.
Step 5
Report Covid-19 vaccine inventory daily to VaccineFinder
In some jurisdictions, providers report daily inventory to the jurisdiction’s IIS, and the jurisdiction reports that information to VaccineFinder on behalf of providers.
Providers will receive guidance from their jurisdiction on where to report their daily inventory of COVID-19 vaccines (i.e., IIS or VaccineFinder).
If your jurisdiction requires you to report daily inventory to VaccineFinder, you will receive an email to begin the enrollment process in VaccineFinder and complete your VaccineFinder profile.
